Pal sickness

Every illness a Pal can catch at your base, what it actually costs you, and which medicine clears it. The medicine grade is not a matter of taste — each illness has a minimum grade in the game files, and anything below it does nothing. The worst, disturbing element, takes 50% off work speed and 50% off movement.

Which medicine to use

Medicine items carry a rank, and an illness carries the lowest rank that clears it. A higher grade always works on a lower-grade illness, so the cheapest correct answer is the one matching the illness’s own rank — brewing High Grade Medical Supplies for a cold is a waste of a Pal’s afternoon.

The one no medicine covers

Disturbing element sits at rank 4, and the game ships no rank-4 medicine — the highest is High Grade Medical Supplies at rank 3. What the table does give you is the fallback: a 3% chance of recovering on its own while sitting in the Palbox, against 20% for a cold. It is the slowest recovery in the game by a wide margin.

Leaving it in the Palbox instead

Every illness has a recovery chance that applies while the Pal is in the Palbox rather than working. It scales the way you would expect — 20% for the mild ones, 15% in the middle, 10% for the serious ones — so parking a sick Pal is a real option for the cheap illnesses and a bad one for the expensive ones. Note that a Pal in the Palbox is a Pal not working, which is the actual cost of the lazy fix.

Prevention is a Medicine Production Pal at the Medieval Medicine Workbench, which turns the whole problem into a background task.

A note on the names

Palworld has no text table naming the individual illnesses — the UI shows a generic “Sick…” label. The names above are the game’s own internal identifiers with the spacing put back (GastricUlcer → gastric ulcer), not names we invented. Everything numeric on this page is stated directly in the game files.
